Our Coordinated Care Program provides critical resources and discounts to help people live with chronic health conditions. Through this program, covered team members and/or dependents are eligible for the following benefits:

  • Free prescriptions related to the three most common chronic health conditions:
    • Diabetes
    • High Blood Pressure/Hypertension
    • High Cholesterol
  • Access to Coordinated Care Managers – a team of caregivers that can provide knowledge, tools and self-care techniques to help you improve your health and live with chronic health conditions.

Get Started

Step 1 Visit a designated Hackensack Meridian Health owned physician practice to receive treatment for the following chronic conditions: diabetes, high blood pressure/hypertension and/or high cholesterol.

Step 2 Fill any prescribed medication related to diabetes, high blood pressure/hypertension or high cholesterol at one of the HMH in-house pharmacies (mandatory in-house pharmacy fill). Covered prescriptions include generic drugs, as well as brand name drugs on the OptumRx Formulary drug list for which there is no generic alternative.

Step 3 HMH’s program provides access to Lumeris Coordinated Care Managers – a team of caregivers that can provide knowledge, tools and self-care techniques to help you improve your health and live with chronic health conditions and assist team members with a specialized care plan.
